1), 2) - problem to upload preview image on our site, not sure what it has common with plugin itself
3, 4) - there was problem to upload some styles, please send me your URL + login to oc-admin (PM login data)

I solved the problem "Adblock" interfered with the plugin!

Solution: disable AdBlock module/addon in browser.
