How do I add-hook of the attached image under the items instead of the single promotion hook, I want users to subscribe for membership not to promote a single listing.

There is not such form prepared, anyway should not be complicated to do it by re-using existing functions. Check i.e. that business profile or maybe category restriction page layout.
