Hello,
I was expecting that if someone pays to make their ad premium, after the payment has expired, the item will automatically lose premium status. But that is not working. I have to manually unmarked it as premium. How can this be addressed. It’s a lot of work checking everyday to find which item has expired premium status.

Premium item is intentionally not expiring in order to provide service that customer paid for.
Once item loose premium label, it expire.

It does not even expire after losing premium time. I am not sure if it has to do with cron job. I use Bluehost on shared hosting. Here is how I set my cron job. Please let me know if I got it wrong:
I did once every hour and I used this url: https://mysite.com/index.php?page=cron

Once listing is not premium anymore and has expired, it will not show in search anymore.
No cron is required for that (btw setup daily as well)

@Cephas may be right. Faced the same issue in the past with premium etc. Including Membership expiration and republish. Items do not republish daily until Cron is configured properly.
After that, even the republish that were supposed to be done in the past and didn't and the time has passed were all republishing.
That was my experience by the way.
You may want to investigate the Cron issue very well to ensure it is actually running. OsclassPay shows the time of the last Cron Job
