There is typo in functions.php of pluign:
?page=plugins&action=renderplugin&file=polls/admin/
...into:
?page=plugins&action=renderplugin&file=poll/admin/

This user's rating is hidden only because they rate the site

I would also like to know if we can remove the survey button after user have canceled or taken the survey. Survey button should disappear from the page. Is there a way to do this? Please suggest.

Currently no way as plugin does not store this info in cookies.
