veronica theme category and category icon problem

i'm using veronica theme when i postadd  to personal category like womwn looking for men it shows two unwated option http://prntscr.com/fh2ouo how to remove it only personal and also when i try to upload the icon image iit is not reflected

You can just hide or show these field in all categories.
