Can anyone tell me how to create custom contact form in veronika theme on new page with many fields like name, email, phone, country, message, etc ?

Please help me

i am not asking for any plugin.

i am asking that how to create custom equiry form on new page in osclass ?

but why create a new one ?? It's easier to add fields that you need to a form that's already there. Besides, there are a lot of forms on google. ;D

Yes there are many forms on Google. But they have limit to send email like 100 per month. I want new form on new page with many fields like dropdown, textarea, phone, email etc
